The Diabetes Education Program at Bramalea Community Health Centre offers comprehensive diabetes education to people living in Bramalea, Brampton and Malton: 

  • Living with Type 2 diabetes
  • At risk of developing diabetes (pre-diabetes)
The program and its services are free of charge and delivered by diabetes nurse educators and registered dietitians.  You can meet them privately or in a group.  To join the Diabetes Education Program click here.

People living with Type 2 diabetes will learn how to:
  • Reduce risk of complications
  • Deal with emotions connected to having the disease
  • Monitor sugar levels and keep within target range
  • Manage your stress
  • Control your weight
  • Prevent foot problems
  • Take your medications correctly
  • Develop a healthy eating plan
  • Meet and learn from other people with diabetes
If you are risk of diabetes you will learn how to:
  • Bring down your glucose levels
  • Design a plan to lose those few extra pounds
  • Learn how to read labels and make healthy food choices when grocery shopping
  • Get moving with a regular and realistic physical activity plan
  • Take your medications correctly
Convenient and multi-lingual
  • Our team can speak to you in Urdu, Punjabi, Hindi, Greek and English
  • If you speak another language we can also provide interpretive services
  • We will provide bus tickets for group sessions (as needed)
  • In coming months watch for the Diabetes Educaion Programs at various locations throughout Brampton, Bramalea and Malton
