Programs and Services for People who are Homeless, at Risk of Homelessness and Under Housed

People who are homeless and registered as clients at Bramalea CHC can access primary health care from a team that includes a nurse practitioner, doctor, registered nurse, dietitian, physiotherapist, social worker and community health educator.  For more information about becoming a client click here.

Other services include: 

  • Primary care from nurse practitioner, physician and/or registered nurse at local shelters
  • Counselling to address a wide range of mental health and substance use issues
  • Support accessing other community based supports such as subsidized housing, shelters and food banks
In addition to offering individual supports to individuals who are homeless, we also work on larger initiatives and partnerships to end social conditions that contribute to homelessness.  For example,
  • Our team has been working closely with the HOPE coalition to coordinate a two day symposium on homelessness here in Brampton.  The symposium will celebrate progress on the 10 year anniversary of the Region of Peel report on Homelessness.  Recommendations will hopefully emerge on further work that needs to be done in the community.
  • We have also connected with the Region of Peel Street Outreach Team to ensure they refer homeless and under-housed people to our primary care team as well as our other programs and services.
