Primary Health Care

Registered clients can access services from a team of health providers that includes:

Nurse practitioners
Family physicians
Registered nurses
Community health educators
Medical office assistants
Social workers
Dietitians
Physiotherapists

Same Day Appointments

We are committed to providing the right care at the right ime. And so we book appointments on the day you want to see us, even if it is the same day you call.

If you have trouble remembering when you should return to see us, let us know and we will remind you.

No Walk-Ins

All registered clients must call to make an appointment. Calling ahead helps us to prepare for your visit and save enough time to take care of your needs.

Shared Medical Appointments

For those who are interested, we also offer shared medical appointments. A shared medical appointment (SMA) is a 90 minute appointment with a family physician or nurse practitioner. This appointment will be held with other Bramalea CHC clients who share similar conditions.

When you attend an SMA you will have the benefit of having access to a group of health care providers as well as the support of other clients who have a condition similar to yours.

Shared medical appointments are especialy effective for clients requiring:

Pre-natal care
Post-natal care for well babies
Care for chronic diseases
Medical check-ups

Anything you would normally do in an individual office visit can be done in an SMA. Examinations are completed privately. You also receive the same exam, lab review, and time for individual questions as you would in a traditional appointment.

What's good about an SMA is that during the 90 minute appointment you have the opportunity to learn from the questions and comments of others who share your situation. You'll leave the appointment with answers to questions you may never have thought to ask.

Most people who try shared medical appointments want to return but you are under no obligation to do so. If you prefer an individual appointment, you may schedule one at any time.
