Programs and Services for Seniors
At Bramalea CHC we are committed to working with the older adult population. This population is defined as individuals 65 or older.
Seniors registered as clients at Bramalea CHC can access primary health care from a team that includes a nurse practitioner, doctor, registered nurse, dietitian, physiotherapist, social worker and community health educator. For more information about becoming a client click here.
Our goal is to break down barriers seniors face accessing the care they need. We'll be providing:
- Home visiting
- Medication management
- Counseling and therapy related to mental health, healthy aging, abuse and other related health matters
We also recognize that isolation is a problem many seniors face They may no longer spend time with family and friends, or be able to participate in activities or groups that reflect their interest or allows them to share their passion and skills.
At Bramalea CHC we want to reduce this isolation so they can make the most of al the great experiences and skills they have to offer. Some of our plans to engage seniors include:
- Monthly brunches
- Media program
- Arts workshops
